You can make a complaint to the Texas Department of Health and Human Services (“TX HHS”) by mail or fax, but TX HHS prefers that complaints are submitted online via the Texas Unified Licensure Information Portal (TULIP) or by calling 1-800-458-9858.

Anyone can file a consumer complaint by going to the Texas Attorney General website (listed below). These complaints are not allowed to be anonymous.
